This book provides physicians with an approach to communicating health risks to facilitate patient decision making. This approach uses the Risk Characterization Theater (RCT) originally presented in The Illusion of Certainty: Health Benefits and Risks, published by Springer in 2007. The RCT is a simple visual aid for communicating with patients: a theater seating chart, which presents a clear and objective picture of health benefits and risks associated with screening tests, drugs, surgery, and other medical intervention. The image demonstrates at a glance that a 1 in 10 risk is very different from a 1 in 100,000 risk, and makes it easy to illustrate the number of people who would benefit from screening tests or drugs; the number of individuals benefiting from surgery; the merits of published risk factors; and so on. The use of the RCT encourages physicians and patients to openly discuss issues such as: Who should make the final decision regarding medical intervention? What risk level is acceptable? Is the decision evidence-based? Do the benefits of a procedure or a drug outweigh the risks? The first part of the book discusses the role and responsibility of physicians when addressing concerns associated with appropriate medical intervention. Other chapters focus on acceptable risks, existing controversy regarding appropriate medical intervention, important differences in characterizing individual and national risks and benefits, effective communication of health risks and benefits, differences resulting from the use relative and/or absolute risks, and charts showing comparable risks. The second part of the book is devoted to case studies. These case studies use the RCT to illustrate the benefits and risks associated with the most common medical screening tests (e.g., colonoscopy, PSA test, mammogram, cholesterol); drugs for cancer and other diseases; and other forms of medical intervention.
